In order to simplify a billing process that involves a multitude of insurance plans, we kindly request that our patients take full responsibility for the payment of their dental services. We can assist you with your reimbursement by submitting your insurance forms on your behalf or providing you with a detailed statement after each dental treatment or series of treatments that is suitable for filing with your insurance company. The statement of services includes the dental code printed next to the procedure, which is needed for your insurance company to process the claim.
